Registration Enrollment Application

Step 1: Complete the Registration Enrollment Application

To electronically enroll your student at NCPS, you can fill out an application online or fill out the form below, which has multiple sections. Once you complete all the sections, you will see a submit button. After submitting the form, you will receive an email with the completed enrollment document.

Step 2: Sign the Submitted Registration Enrollment Application

You can print the completed enrollment document you received in your email in order to sign, or the school can print this document for you to sign. The signed document will need to be turned in at the anticipated school building along with the required documents listed below. If you have any questions, you can call the school for assistance.

Step 3: Provide the School with the Required Documentation

  1. Proof of Residency (2 Verifications of Physical Address)

    • Lease agreement or rent receipt

    • Mortgage contract or payment receipt

    • Utility bill (water, gas, electric, cable, or landline phone)

    • Personal property tax bill or receipt

    • Letter from Social Services or Social Security

    • Bank Statement, etc.

  2. Proof of Mailing Address - if the mailing address is different from the physical address.

  3. Medical information that includes immunization records, as well as physical examination (if transferring from out of state).

  4. Original Birth Certificate or Certified Birth Certificate.

  5. Proof of Guardianship - proof that the adult registering the child is the parent/legal guardian.

    • The name on the birth certificate should match the parent/guardian's picture ID or court

      documents of legal custody.

  6. Official school records from another school system or country, if applicable and available.

NOTE: A driver's license or state-issued ID does NOT serve as valid proof of residency. If you reside with someone else, you will be required to complete the Homeless Affidavit Form and submit 2 documents of verification of the householder.
